3． 俄罗斯登记证件（移民卡）。
4． 公证员出具证明的俄语译本的证件。
5． 医学服务证件（需要收费的）以及收据。

Cost of the service: 2000 roubles, plus PCR diagnostics on COVID-19 -1500 roubles.

List of the documents, required for the medical examination:

Personal identification document of a foreign resident — passport of a foreign citizen.

Personal identification document of a person without citizenship – temporary residence permit; permanent residency; a document issued by a foreign state and recognized by the Russian Federation as a personal identification document in accordance with an international treaty.

Document confirming the right to remain in the Russian Federation.

Document confirming migration registration (migration card).

Russian notarized translation of the personal identification document.

Contract for commercial medical services and confirmation of their payment.

It is necessary to have originals and copies of the documents specified in points 1 — 4.
